UO ID Badge
Some UO employee services (e.g. free bus rides, library facilities, computer labs, event admissions) require a UO ID card. You can obtain a card from the UO Card Office, located on the ground floor of the Erb Memorial Union (EMU) at the corner of 13th and University. This office is open from 8:00 to 4:30, Monday through Friday.  For more information call (541) 346-3113.
You will need a photo identification (driver’s license or passport) with you as well as a photocopy of a completed PIF (Personnel Information Form). Departmental payroll staff (Birgit Musheno or Colleen Maas) can provide you with a copy of the PIF form. Replacement cards can be obtained with photo identification and payment of a $20 fee.
Practicing Professional Policy & UO ID Badge
Supervisors Badges indicate that the supervisors have received a background clearance. The UO College of Education (COE) has a centralized clearance / background check process for University Supervisors to obtain their Faculty Badge. We require proof of clearance for everyone, but it’s a fairly simple process. If you are a currently licensed teacher, you do not need to do the finger print process and can jump to step 2, uploading documents.
Step Who What
1 University Supervisor The University Supervisor creates an account on the Teacher Standards and Practices Commission’s (TSPC) eLicensing system beginning with the tutorial and directions at http://www.oregon.gov/tspc/Pages/elicensing.aspx
University Supervisor The University Supervisor submits an application and payment through the eLicensing (https://apps.oregon.gov/TSPC/eLicense) system on the TSPC website for background clearance for student teaching or practicum.
License category in eLicensing = Clinical Practices
Cost = $55.00, plus $10 portal provider fee
ETA = TSPC will email codes and instructions within 5-7 days.
TSPC Important notice: The University Supervisor will receive an email from TSPC, which contains codes and instructions the applicant needs to proceed, within 5-7 days. TSPC emails instructions to the applicant for accessing Fieldprint. The email will consist of the following required information specific to the applicant::
Fieldprint Agency Code: XXXXXXXXXXXXXXXXXX
Billing Code: xxxxxx
ORI: xxxxxxx
OCA: Txxxxxxx (TSPC account ID)
University Supervisor After TSPC emails the required codes to the applicant: The applicant schedules a fingerprinting appointment through the Fieldprint website and pays the required fee (currently $12.50).
Fieldprint After the fingerprinting process is complete, Fieldprint sends an image of the prints to the Oregon State Police (OSP) for processing through both the Federal Bureau of Investigation (FBI) and Oregon State Police databases.
OSP OSP forwards the results electronically to TSPC. This process is taking approximately two weeks, with some clearing faster and some taking longer.
TSPC TSPC records the results as part of the application process. It is the applicant’s responsibility to monitor their record to determine when the background check results are complete.
2 University Supervisor Background check results are available on TSPC’s Educator Lookup (http://www.tspc.oregon.gov/lookup_application/lookup_query.asp) website after about two weeks. All requested information (first name, middle initial, last name, birthdate and last 4 of the Social Security Number) must be entered to see the full details of the TSPC record, including clearance date and TSPC ID number. After the applicant’s background check has cleared, the date of clearance is displayed. You will need to make an electronic copy of the clearance screen to upload. TSPC may not send any notification that clearances have posted, so be sure to check back!
3 University Supervisor

Upload proof of your clearance at https://goo.gl/DuHCcd.  Be sure to answer the accompanying questions on the first page completely, then upload confirmation of your clearance on the second page.

Clearances must be uploaded PRIOR to entering schools for field experiences.

4    Go to EMU Card Office to purchase your new Practicing Professional card

There will be a $7 charge. In addition, supervisors must comply with the policies and procedures of the school district. The COE ID  badges is only valid for three years.
