Academic Preparation Forms
As an applicant to UOTeach, you are required to complete an academic preparation worksheet for the level (Elementary or Mid-High) and the primary content area you are applying. Please click the link and download the appropriate form to your computer. There we ask that you read the form carefully, note that all applicants will be considered for admission, regardless of gaps in the form.
To fill out the form, copy and paste courses directly from your transcript(s) into the word document or fillable pdf form. Course and grade information should match your transcript. Upload your complete form to your application in Slate. Be sure to check you have upload your form correctly in the Slate application.
